Cardiomyopathy of recent onset

Cardiomyopathy of recent onset, EF < 50 %, NYHA class II-IV

Etiology

  • Ischemic (non invasive testing, coronarography)
  • Rhythmic (ECG, Holter monitoring, ET)
  • Alcool or drugs
  • Drugs (chemotherapy, antidepressive agents, phenothiazines, anti TNF)
  • Infectious (VIH, Borreliose, Cocksakie, Chagas, jellyfish, snakes, scorpions)

Treatment

  • IEC (sartans) + diurétiques si rétention hydrique
  • ß-blockers for heart rate > 70 bpm m
  • ICD if EF < 30 %

Persistance of symptoms

  • Eplerenone (Inspra) or spironolacton (Aldactone)
  • Ibravardine (Procoralan) if HR > 70 bpm
  • Pacemaker CRT if QRS > 120 ms
