Welcome to this interactive ECG course.

Although it is one of the oldest paraclinic exams, dating back to the late 19th century, the ECG is still of crucial clinical use. This examination often still poses problems of interpretation to the medical practitioner.

This course aims to help the student, the practicing physician and even the trained cardiologist to improve his knowledge in electrocardiography. It consists of 250 traces of varying complexity with a description of each one by experts. This allows the reader to compare his analysis with that of the experts. In addition, the areas of interest of the ECG can be activated to be clearly highlighted.

We hope that these plots will be useful to readers and will improve their knowledge.

Description

Baseline rhythm

Broad complex tachycardia at 145 bpm.P-waves: visible before the QRS.PR interval: normal.QRS: broad with a duration of 140 ms. Axis -40 degrees.ST segment: non-analysable.T-waves: very peaked in III, aVF and in the precordial leads.QT interval: normal.


Zones

Wide QRS complex tachycardia.
Flutter waves with 2:1 conduction.

Diagnostic

Atypical atrial flutter with 2:1 conduction and LBBB.


Comments

We are looking at a broad complex tachycardia. The rate in the region of 150 bpm should immediately make one think of flutter with 2:1 conduction. The T-waves are peaked because they encompass a flutter wave. In addition, in II, III and aVF, we see the ‘saw-tooth’ or ‘factory roof’ appearance of the flutter waves of classical flutter. As for the QRS, it is of typical LBBB pattern.
